Journey Hospice Advances to Partner Level Three Distinction in We Honor Veterans Program
The team at Journey Hospice proudly announces that it recently achieved Partner Level Three in the We Honor Veterans program. Established by the National Hospice and Palliative Care Organization (NHPCO) in collaboration with the Department of Veterans Affairs (VA), the program assists in creating Hospice-Veteran Partnerships (HVP) between VA facilities, community hospices, state hospice organizations, and others working together to ensure that excellent care at the end of life is available for our nation’s veterans and their families.
We Honor Veterans provides tiered recognition to organizations, known as Partners, that demonstrate a systematic commitment to improving care for Veterans both within their organizations and the communities they serve. Journey Hospice began its partnership with We Honor Veterans in 2019. To reach each level of distinction of the program, Journey Hospice delivers educational programs for veterans and their families, assists with resources regarding benefits and coordination of care, and honors numerous veterans throughout South Jersey. Journey Hospice is one of 73 hospices in New Jersey to be a We Honor Veterans’ partner, and one of only 10 in New Jersey to attain the Level Three partnership.
In 2021, Journey Hospice began offering Veteran Cafés for local veterans to come together to socialize and reminisce about their days of serving our country. The cafés are facilitated by a local veteran who is a Journey Hospice veteran volunteer. Veteran attendees share conversation and camaraderie as well as access valuable resources for themselves and their families. Over the past two years, the team has also hosted a Veteran’s Day Salute, an End-of-Life Care presentation, Flag Day ceremonies, Memorial Day and Labor Day events, as well as numerous veteran recognition ceremonies at various South Jersey assisted living communities.
As the staff and volunteers at Journey Hospice work towards attaining the next We Honor Veterans partner level, additional services and resources will be developed for veterans and their families, new VA community relationships will be fostered, and more veterans will be recognized for their service to our country.
